So.Re.Sa. is the Regional Healthcare Company of the Campania Region created to implement strategic interventions aimed at optimising regional healthcare spending. Through the Sinfonia platform, So.Re.Sa. guides the digital transition of public health processes, improving the efficiency and quality of services offered.

The Campania Region and So.Re.Sa. have identified the need to undertake the development of their own computer system for the Management of Rare Diseases, operational since 2012, in order to improve efficiency and ensure Campania's independence from the system currently in use, developed by the Veneto Region.

The intervention aims at both technological evolution and optimisation of functionalities, which have evolved over time thanks to the experience gained in the field.

The new system represents a strategic step towards strengthening regional healthcare governance, aligning with the goal of full integration with theSinfonia digital ecosystem.

A microservice approach for regional healthcare

The solution devised by Exprivia in full synergy with SoReSa SpA, resulted in an information system based on a microservices approach.

This modern and efficient architectural pattern overcomes the limitations of traditional monolithic applications in numerous aspects.

  2. Simple integration: communication with other systems via REST APIs
  3. Functional autonomy: exclusive control over own data and domain
  4. Release autonomy: CI/CD pipeline, rapid time-to-market
  5. Technology autonomy: freedom of choice in technologies, programming languages and databases
  6. Vendor Independence with JPA and Hibernate: independence from the database vendor, avoiding lock-in and favouring flexibility and portability
  7. Integration with SPID and OAuth: shared login with the rest of the ecosystem

      The system is based on Spring Boot and Angular frameworks, software industry standards, and on a containerised infrastructure with Docker and Kubernetes for flexible and efficient management.

      The management of rare diseases represents a crucial challenge and a highly valuable opportunity for the clinical sector. To meet this need, Exprivia, in collaboration with SoReSa, has developed an innovative Rare Disease Management System, designed to ensure an efficient, patient-centred approach that fully complies with ministerial and regional guidelines.

      This system introduces the concept of Presa in Carico, designed for patients who wish to pursue their clinical pathway with the involvement of several specialists. The approach allows the principles of precision medicine to be applied, tailoring treatments to the specific characteristics of each patient.

      Designed to integrate seamlessly with the existing healthcare infrastructure, Sinfonia, this system not only ensures uniform data management and sharing across the entire healthcare ecosystem, but also guarantees full adherence to current regulations. Inspired by the operational best practices of the Veneto Region, the new application combines continuity in processes with an innovative, advanced and challenging structure.

      The new system also supports the systematic collection and recording of rare disease cases within the network, facilitating thestatistical analysis of data, patient monitoring.

      With this innovative platform, Exprivia has created an infrastructure that fully meets the needs of healthcare professionals and aims to radically improve the patient experience. The transition to the new system has provided the Campania Region with an autonomous solution, capable of responding precisely to its specificities, laying a solid foundation for a future of excellence in the management of rare diseases

      System Expansion, Territoriality and Business Intelligence

      The project represented a decisive step in the management of rare diseases in Campania, paving the way for concrete improvements and offering new and promising future prospects. Despite the significant progress already achieved, there are ample opportunities to further evolve the system, making it even more sophisticated and efficient.

      Among the objectives currently being analysed and planned, the integration of the system within the diagnostic and therapeutic processes stands out, a fundamental step which, once completed, will make it possible to optimise every single phase of the care pathway, guaranteeing patients increasingly precise, personalised and timely support.

      At the same time, the strengthening of territorial networks, which has already been started and is being developed, represents an extraordinary opportunity to guarantee a truly capillary and efficient management of rare diseases, providing patients with accessible and widespread care throughout the territory. No less important is the creation of a structured communication flow between doctors and specialists, an ambitious challenge that is already in the planning stage. The implementation of such an interconnection system is essential to ensure a multidisciplinary approach of the highest level, which is crucial to address the complex needs of rare diseases, often characterised by multi-system implications.

      This approach will have a direct impact on the quality of care, ensuring treatments that are more integrated, effective and responsive to patients' real needs.

      Furthermore, thanks to BI - Business Intelligence systems, the collection of diagnostic data will enable the identification of patient clusters and the generation of specific indicators. Through the construction of advanced dashboards and reports, it will be possible to immediately intercept relevant clinical trends, supporting care planning and optimisation.

      Finally, the project, with its future developments, lays a solid foundation for an increasingly advanced and innovative management of rare diseases, improving the quality of life of patients and optimising the resources of the healthcare system.
